2025 Track & Field Registration - MSHSL Forms to PRINT

***This is the paperwork-only portion of your registration.***
You must also complete the Nighthawks online Family Registration forms, roster your athletes,
and pay your fees to hold your spot on the team.
Instructions:
- Print these instructions and read each step to help you fill out the forms correctly.
- Print everything SINGLE SIDED, sign, and complete ALL fields on ALL pages requested.
If you experience distortion in printing--common with non-PDF files--you must first save the file to your computer, then print. - Read through to the bottom of the page so you don't miss any instructions.
- All paperwork (including medical forms*) must be completed and submitted at the team meeting to secure your spot on the roster and avoid late registration fees.
- To avoid the late fee ($50):
- Missing/incomplete forms must be submitted by midnight after the team meeting.
- Errors: The Registration Team will double-check details and contact you within 2 days of the meeting to correct any errors (e.g., missing signatures). You will have 2 days to submit corrected forms. After this, $50 will be invoiced to the family account.
- If 7 days after the meeting forms are missing/incomplete, those student-athletes may be moved to the waiting list without a refund.

1. 2024-2025 MSHSL Activity Registration Form<--Click & print from the folder.
- One form per family.
- Check this season's sport only--no other sport/activity.
- "Name" = Parent's name
- "Position" examples include "Head of School," "Principal," etc.
2. Be prepared to sign the MSHSL group documents (see below).

9th graders: Contact us to determine eligibility before filling out forms.
High school (grades 10-12): 5 forms required (1, 2, 3a&b, 4, 5) plus MSHSL group documents (#6)
1. Proof of Home Schooling
Copy of your 2024-25 Homeschool Reporting Form ("Initial Report: or annual "Letter of Intent to Continue") as sent to your resident school district (see example here).
2. 2024-25 Sports Qualifying Physical Exam Clearance Form<--Click & print if we do not have yours on file.
- Physicals must be dated within 3 years of the start of the sport season.
If it will expire during the season, you must submit a new sports physical at the team meeting. - Any medical considerations requiring additional paperwork are due at the team meeting.
(Example: Asthma Action Plan <--Click & print if you will be using an inhaler--must include doctor's signature releasing use of medication)
Must be dated within 1 year of the start of the sport season. - Exception: First-time team families may submit their medical paperwork up to 4 weeks after the team meeting. However, at registration they must provide a sports physical appointment date to avoid late fees. Student-athletes may only participate in pre-season (summer, captain's practices, etc.) runs if they have submitted the required medical forms
- Medical forms are retained in NHAC files but not sent to the League.

Submit only pages 10, 11, & 12:
(a) MSHSL Eligibility Statement (page 10-11). Be sure to check the appropriate "YES" or "NO" boxes for the two questions at the bottom of the page ("I am a homeschool student?" and "I am an online student?"). Students enrolled in online public schools (e.g., Harmony Education, MN Connections Academy, etc.) are ineligible for participation.
(b) MSHSL Annual Sports Health Questionnaire (page 12). Fill it out, and be sure both the parent and student sign it.

5. 2024-25 MSHSL Home School Resolution for Membership<--Click & print from the googledrive folder.
Please submit contact information to be publicly listed in the online MSHSL membership directory.
6. Be prepared to sign the MSHSL group documents.
Parents of high school students must also read (no need to print) the following group documents and be prepared to sign at the team meeting:
- MSHSL Application for Cooperative Sponsorship and Application for DISSOLUTION of Cooperative Sponsorship
(Samples; there will be separate applications for boys and girls. The MSHSL requires a dissolution of a cooperative sponsorship one year before allowing a new cooperative sponsorship to be formed. To meet deadlines, we all complete both forms at the same time.) - 2024-25 Cooperative Sponsorship Agreement
(This link is a sample; there will be one agreement for the boys' team and one for the girls' team, to form athletic teams among homeschool high schools with students in grades 10-12. Note: MSHSL agreements apply to all siblings of the same gender in each family. This agreement is for track & field only. This does not mean your students have to participate on the track & field team next year, it just means that they cannot bounce from one track & field team to another each year. The same applies to public/private school students.)
High school (JV/Varsity) participants and parents are also responsible for the information contained in the following:
- MSHSL homeschool membership rights, fees, and MN State Statutes
- General MSHSL homeschool info
- MSHSL Handbook and website
- Athletic Director/Coach Jennifer Swanson will be your school's eligibility representative at the mandatory annual Area Meeting.
Most forms are retained in NHAC files and not sent to the MSHSL. This includes the sports physicals, annual health questionnaires, and MSHSL Eligibility forms. The following forms are sent to the MSHSL on behalf of high school league members: proof of homeschooling, MSHSL membership and activity registration forms, and cooperative sponsorship and dissolution applications.
We (NHAC) and the MSHSL will do everything we can to alert you to deadlines, but ultimately you are responsible to keep your MSHSL membership and your student-athlete's eligibility in good standing.
